As part of the wider Cardiovascular Disease Prevention Programme, Health Innovation North West Coast is delivering local projects to reduce hypertension and encourage home blood pressure monitoring (HBPM), supporting nationally driven initiatives to encourage patients to remotely monitor their blood pressure.
This helps reduce impact on resource pressures in primary care and enables people to be more active in the own healthcare.
Aims
We work collaboratively with partners to encourage and support regular monitoring of BP at home whilst being conscious of not widening healthcare inequalities.
Delivery in the North West Coast
In this edition of the Health Innovation North West Coast podcast Caroline Kenyon talks to Jane Briers who is leading the blood pressure at home programme for the Health Innovation North West Coast. They're joined by Dr Jonathan Griffiths, a GP in Cheshire and clinical lead for Cheshire and Merseyside for the BP at home project, who explains how the programme is already paying off.
Blood Pressure Quality Improvement
The Blood Pressure Quality Improvement (BPQI) Toolkit is a collaboratively and locally developed QI package that embeds into EMIS and System One and supports general practice teams to deliver best practice through dashboards, consultation templates and more. Health Innovation North West Coast has a dedicated team supporting partners in the North West Coast to improve management of hypertension.
