This area of the site is a resource for community pharmacists and their teams, to gain information about TCAM/ e-TCP.

 

You will find videos, step-by-step guides and handy guides on using the PharmOutcomes system. This is the system that all Cheshire and Merseyside trusts have chosen to use. The resources can be downloaded, printed and shared within pharmacies across the North West Coast.

In addition to the site, a number of events are being held to provide face to face training on the practicalities of using the interface. We will let you know when there is an event in your area.

To help ensure that referrals are actioned in a timely fashion by community pharmacists, you can obtain a device called PharmAlarm

The PharmAlarm provides a visual indication in the pharmacy whenever a hospital discharge referral or other notifications such as NUMSAS is received in PharmOutcomes. This avoids the need for pharmacists to log in to the system to check if a discharge referral has been received and makes the overall use of the PharmOutcomes more user friendly and interactive.

MANAGING A REFERRAL

 

 

Not all trusts are live yet and at least one of the trusts across Lancashire are using an alternative system such as Refer2Pharmacy. If this is adopted wider, the AHSN project team will support the community pharmacists to understand this interface.
